I just quickly looked over this and noticed a few things that will definitely not allow this theme to approved. This is not a full review of the theme, but you need to fix these things before anyone will do a review of your theme.

• Multiple credit links in the footer are not allowed by default. See: ​http://codex.wordpress.org/Theme_Review#Credit_Links
• The "Code" font is not GPL-compatible, thus not allowed here.
• Much of your /includes/post-formats.php code is missing the proper copyright and license notices. You can get them from here: ​https://github.com/justintadlock/hybrid-core/blob/master/extensions/post-format-tools.php
• Theme URI not appropriate. See: ​http://codex.wordpress.org/Theme_Review#Credit_Links

I didn't check all the code, but make sure you're properly adding the copyright and license notices for all third-party resources in your theme.
