My Notes:

At the first sight Author URL - ​http://halloweenlove.com looked like a commercial site, but it's actually a Blog about Halloween stuff, in my opinion acceptable.

• Author URI, if used, is required to link to an author's personal web site or project/development website. It's personal, regardless of the ​Zazzle Store link.

Few details we need to address before Theme goes live:

• "Theme: Haunted House" When text like this is used we need either to point this to e.g. example.com/haunted-house-theme or change that to "Halloween Love". Also see: ​Credit Links.
• Background image (the house) doesn't have any copyright information. Please note where was this image acquired from.

Reassigning back to you, please let's help and get this reviewed quickly. It would be nice to offer 2-day grace period before Theme gets closed just so that the author can fix requirements and re-submit.

Yup, mostly all free Halloween fun and goodies; icons, wallpaper and now a WP theme for Halloween lovers. There's some side stuff to try and pay the bills, but a necessary evil of most blogs these days.

• I'll change credit to "Theme By: Halloween Love Network" (which is the most official name) if that seems most appropriate.

• Actually, the credit for the image is marked. Must of just missed it.
